Dear New Applicant,

Northside Hospital Forsyth will be offering a VolunTeen Program this summer for high school students between the ages of 16 (must be 16 by January 1 the year the teen applies) and 18 years of age.

The objectives of our VolunTeen Program are to conduct an excellent education program to develop an interest in volunteer service for our participants and to provide an opportunity for students to experience the total healthcare environment. The teens accepted for membership in the VolunTeen program will be trained and supervised by either a Northside Hospital Forsyth Auxiliary member or a hospital staff member.

Continuity is vital to our program. Do not apply to our summer program if you plan to miss more than one shift during the six-week program or cannot attend the first week of the program.

Submit an application only if you can:

1. Meet the required age requirement (must be 16 by January 1).
2. Attend the entire mandatory orientation.
3. Commit to volunteering for the full six-week period.
4. Attend your first volunteer assignment the first week of the program.
5. Attend the Lunch and Learn event during the summer session.
6. Wear the appropriate uniform at all times while volunteering.
7. Limit your absence to only one absence during the six-week session.

Instructions for submitting required forms for new applicants:

All documents must be mailed by the deadline date (see Schedule Timeline) for consideration to be accepted in the program. Forms dropped off, scanned or faxed to the Auxiliary Office will not be processed.
