The Northside Hospital Forsyth Auxiliary Scholarship is being offered to students currently enrolled in a MEDICAL field of study or a Health related degree program at an accredited college, university or technical school.

To QUALIFY a student MUST:
Be a full or regular part-time Northside Hospital Forsyth employee or family member of an employee who has completed a probationary period

OR

Be a current Northside Hospital Forsyth Auxilian or family member of an Auxilian who has served at least 100 volunteer hours annually.

Scholarship Funds are to be used ONLY for tuition, books and school fees.

1. PRINT the Application and COMPLETE every blank. You may use N/A when appropriate.
2. The information below (A B & C) must be included with your Application Packet. Do not send the application and the forms separately!
A. Provide TWO letters of Recommendations from a teacher, counselor, work supervisor or clergy on his or her LETTER HEAD. The recommendation must be placed in a “sealed” envelope.
B. Provide an official, sealed TRANSCRIPT(s) from accredited college(s), university(ies) or technical school(s). (NOT A PHOTO COPY OR A FAXED COPY)
C. Submit a ONE PAGE “personal narrative profile” of yourself noting experiences that are pertinent to your career goals. Please include extra curricular interests, sports, volunteer activities, part-time or full-time employment.
3. Be available for a personal interview with the scholarship committee. The interview will be scheduled after receiving the completed application and forms.

THE ELIZABETH WILMOT BULL SCHOLARSHIP APPLICATION is NOT online. It is “ Included" in the packet you will pick up at the front desk. If you plan to submit the EOB Application be sure to pick up your packet in time to meet the DEADLINE below:
