The 2023/2024 Scholarship application forms will be available online or in the Volunteer Office in October 2023.

The 2023 Scholarship recipients were awarded a total of $24,000.

The Northside Hospital Forsyth Auxiliary Scholarship is offered to students currently enrolled in a MEDICAL field of study or a health related degree program at an accredited college, university or technical school.

To QUALIFY a student MUST:
Be a full-time or regular part-time Northside Hospital Forsyth employee, who has completed a probationary period or a family member of an employee.

Or

Be a current Northside Hospital Forsyth Auxilian or family member of an Auxilian who has served at least 100 volunteer hours, annually.

Scholarship funds are to be used ONLY for tuition, books, and school fees.

Read and Follow all Instructions

1. PRINT the application and COMPLETE every blank. You may use N/A when appropriate.
2. The information below (A, B & C) must be included with your application packet. Do not send the application and the forms separately!
   1. Provide TWO Letters of Recommendation from a teacher, counselor, work supervisor or clergy on his or her LETTERHEAD. The recommendation must be placed in a sealed envelope.
   2. Provide official, sealed TRANSCRIPT from accredited college, university or technical school.
   3. Submit a ONE PAGE personal narrative profile of yourself noting experiences that are pertinent to your career goals. Please include extra-curricular interests, sports, volunteer activities, part-time or full-time employment.
3. Be available for a personal interview with the Scholarship Committee. The interview will be scheduled after receiving the completed application and forms.

THE COMPLETED APPLICATION AND FORMS MUST BE RECEIVED

No Later Than Monday March 4, 2024. There are no exceptions to this date.
